What we do

Care where the patient is

We become the patient's primary care team and see them at home, at their facility, or by video.

In-home visits

Primary care visits at home, in assisted living, or in a skilled nursing facility.

Telehealth

Video visits for follow-ups and check-ins, so patients and caregivers don't have to travel.

Evenings & weekends

After-hours and weekend visits for problems that can't wait until Monday but aren't emergencies.

Who we serve

For the patients who are hardest to reach

Our model is built for patients whose health makes regular office visits difficult, and for the families and doctors who care for them.

  • Homebound or bedbound patients
  • Patients with frequent ER visits or hospital stays
  • Patients just discharged from the hospital
  • Residents of skilled nursing, long-term acute care, and assisted living facilities
  • Patients with multiple chronic conditions who need ongoing care management

How it works

Getting started is simple

  1. Reach out

Call us, or ask the patient's doctor, hospital or facility to send a referral.

  2. We confirm eligibility

We check that the patient lives in our service area and has Original Medicare, and gather recent medical records.

  3. First visit at home

We schedule the first visit at the patient's home or facility, then follow up in person or by video.
