A mobile primary care practice built for patients who can't easily come to us.
Our approach
For many older and medically complex patients, simply getting to a doctor's office is the hardest part of staying healthy. Missed visits lead to emergency room trips and hospital stays that could have been avoided.
Harmony Medical Mobile Clinic brings primary care to the patient: at home, in their facility, or by video. We stay closely involved, coordinate with families and specialists, and are available after hours and on weekends.
We currently serve patients with Original Medicare in the San Gabriel Valley.
