In-home primary care
Regular visits at the patient's home or residence, including assisted living and skilled nursing facilities. We become the patient's primary care provider and coordinate with their specialists.
Telehealth visits
Secure video visits for follow-ups, medication reviews and check-ins. In-person visits are always available if you prefer.
After-hours & weekend care
Evening and weekend visits for urgent but non-emergency problems, to help avoid unnecessary trips to the emergency room.
Transitional care after the hospital
Close follow-up in the weeks after a hospital or facility discharge: reviewing medications, checking on recovery, and coordinating next steps.
Chronic care management
Ongoing support between visits for patients living with long-term conditions, including care planning and coordination with family and other providers.
Facility-based care
Primary care rounding for residents of skilled nursing, long-term acute care and assisted living facilities.

We currently accept Original Medicare (Medicare Fee-for-Service) only. We are not able to accept Medicare Advantage or other insurance plans at this time.

Does the patient have to be homebound?
No. We focus on patients who have a hard time getting to a doctor's office, including homebound and bedbound patients, people recently discharged from the hospital, and residents of assisted living and skilled nursing facilities.
What insurance do you accept?

Will Harmony Medical become my primary care provider?
Yes. With your agreement, our team becomes your primary care provider. We request records from your prior doctors and coordinate with your specialists.

Is this an emergency service?
No. If you have a medical emergency, call 911 or go to the nearest emergency room.
